Maryland Partnership for Prevention will be administering the flu vaccine in the HCPSS elementary and middle schools this year.  Flu Mist WILL NOT be offered.  The vaccine will be given as a shot (injectable). If you know that your child does not do well with shots, please consider accompanying your child.

The CMS flu shot clinic will be held on Wednesday, November 30th.  The consent forms will be sent home the first week of school and will need to be returned immediately.  Consents received after September 9th will not be accepted.  If you wish for your student to receive the flu shot at CMS, please complete the consent form entirely and return it by September 9th.
