This form is for parents/guardians of CMS students to share a question or concern. Submitting this form is a request for review, not a confirmation of change. Only a parent or legal guardian with the right to make educational decisions for the student may submit this form.
We will attempt to honor parent requests, and please be aware that not all schedule requests may be possible. Requests will be evaluated based on criteria such as course availability and class size. Schedule change requests made due to teacher preference are unable to be honored.
